PREPARING FOR APPOINTMENT

Driver's License or State ID
- Bring to every appointment, even if you are currently a patient -
Health Insurance Card
- Bring to every appointment, even if you are currently a patient
- If you do not have health insurance, bring your most recent paycheck stub or proof of income to apply for the Sliding Fee Discount Program. -
Current List of Medications
- This includes any over the counter medications, herbs, vitamins, or supplements you take
- Dosage for each and how often you take them
- If you prefer, you can bring the bottles to your appointment -
Personal Health History, including:
- Current and past problems and treatments
- Discharge instructions from current or recent Emergency Room or hospitalization
- Immunization Records
- Any past medical records or test results you have -
Family Health History, including:
- Diseases that run in your family
If you are meeting with our Enrollment Specialist to enroll in health insurance, be sure to bring your Social Security Card or have your number with you.
