How to Enroll
Patients will be taken to the enrollment form at intake.osfoncall.org and receive a phone call from the OSF OnCall team to complete enrollment.
The patient will receive an enrollment link from Get Well Anywhere, which will be delivered via email. If you do not see the email, please check your junk folder. Please follow the instructions in the email to activate your account. If you do not receive an email link within 12 hours, contact the OSF OnCall team at (844) 381-3603.
