Licensed Clinical Social Worker
We have an exciting new position at Eagle View Community Health System for a Licensed clinical social worker team member to provide assessments, brief encounter or traditional therapy, referrals to outside agencies, and crisis intervention to insured and uninsured patients in a rural health center located in Oquawka and Stronghurst, Illinois (Henderson County).
Training and experience in evidence-based intervention and health psychology required. Candidate will work closely with the medical and dental providers at Eagle View Community Health System to deliver integrated and coordinated behavioral health care in the rural clinic setting. The Behavioral Health Team will be comprised of the Clinical Director, Licensed Clinical Social Worker, Dental Director, Quality Coordinator, and an RN; the team will be supplemented by other support staff as deemed necessary by the Clinical Director and/or Executive Director.

Performs assessments and tests using evidence-based diagnostic tools for patients referred by internal and external sources, for example depression, anxiety, suspected or confirmed substance abuse, trauma (including PTSD), dementia, and other abuse. At times this may necessitate warm hand-off between departments.
Provides brief and traditional behavioral health interventions for non-Seriously Mentally Ill (SMI) patients, including substance abuse assessments and crisis interventions. Provides individual, family, group, and couples therapy. Provides services to assist or refer out all age ranges.
Refers patients with SMI to the appropriate emergency or community services as appropriate.
Participates in management team, full staff, and behavioral health team meetings and any others as required by clinic director or Executive Director.
Participates in chart reviews with the clinic medical/dental staff and serves as a consultant to medical/dental staff on behavioral health matters.
Works with clinical director to plan and conduct in-service training and orientation programs regarding behavioral health matters.
Works with clinical director to plan and participate in the annual educational seminar (yearly) as well as any other community educational outreaches as deemed appropriate or needed after conferring with medical/dental directors/staff and Executive Director.
Provides liaison/outreach networking with multiple area community organizations involved in behavioral health activities.
Maintains electronic health records and reports in accordance with EVCHS policies and procedures.
Acts as a preceptor for social work students requesting practicum experience at EVCHS.
Qualifications, Education, and Experience
The preferred candidate will meet or exceed the following requirements:
A Master's degree in social work with ability to obtain licensing in the state of Illinois.
Two-three years of practical experience in the delivery of behavioral health services is preferred (experience in a Federally Qualified Health Center is a plus), but would consider a new graduate with excellent or outstanding academic achievement supported by letters of recommendation from instructors, dean, and authority figure from practicum experience.
Must be (or able to obtain) LCSW license in Illinois.
